Membership Instructions for Boy’s/Men’s Officials
You will need Adobe Acrobat Reader to view / print form if you don't already have installed.

If you wish to be a WMLOA member as a Boy’s/ Men’s Official you will
need to follow these instructions. Please follow the instructions carefully.
WMLOA members will receive priority in game assignments over non-WMLOA officials.
It is doubtful that non-WMLOA officials will receive game assignments except in emergency
situations.

Verify your US Lacrosse membership as an OFFICIAL, or as a cross participant (one of
the cross participant designations must be as an OFFICIAL). This membership must be valid through the end of the current
year.

Complete the WMLOA membership form completely including the US Lacrosse
membership #.

Enclose a check for $35 made payable to WMLOA along with the form.

Attendance at a rules interpretation meeting of the appropriate level and passing
the appropriate level officials test is required. Refer to the
calendar of events to find the appropriate interpretation meeting and test date.
WMLOA officials must wear the proper uniform as mandated by National
Federation rules.

For NEW
Officials (Joining WMLOA and US Lacrosse as a Women’s Official
for the First Time)
Instructions for New Women’s Officials:

Fill out your WMLOA membership form, leave the US Lacrosse membership # space
blank. Make out a check for $35 made payable to WMLOA.

Fill out the US Lacrosse membership form, check either the box for an official (if
that is the only category that you wish to be classified) or the box for a cross participant. If checking the cross
participant box, then check all of the participant boxes that apply. Be sure to write in your date of birth (DOB).
Check the appropriate spaces next to each participant type. Make certain to check the space that says “ women’s
official” or “both”. Be sure to have a parent or guardian
sign the proper spaces on the form.

Make out a check for the
appropriate US Lacrosse fee, this
check must be made payable to US Lacrosse. If
only joining as an official only, the check should be for
$50. If joining as a cross participant 18 years of age or
older, the check should be for $65. If joining as a cross
participant and under 18 years of age, the check should be
for $45.

Mail the WMLOA membership
form, the check made payable to WMLOA, the US Lacrosse
membership form, the US Lacrosse check, and a self addressed
stamped envelope to:

WMLOA will then forward the
US Lacrosse membership forms for new officials to US Lacrosse
along with a recommendation letter for each WMLOA applicant.
When confirmation is sent back to WMLOA, WMLOA will send each
applicant a confirmation of US Lacrosse membership using the
SASE.
For Returning Officials and Previous US Lacrosse Women’s Officials
